List of Requirements

  1. Letter of Intent / Application Letter:
  2. Fully accomplished Personal Data Sheet (PDS) with a recent passport-sized picture (CS Form No. 212, Revised 2017), which can be downloaded at www.csc.gov.ph; 
  3. Photocopy of certificate of eligibility/rating/license (Preferably but not required);
  4. Photocopy of Diploma and Transcript of Records. (if any) Interested and qualified applicants should signify their interest in writing. 

 

Vacant Regular positions as of January 2025

Interested and qualified applicants should signify their interest in writing. Attach the following documents to the application letter and send to the address below:

  1. Letter of Intent indicating the position applying for;
  2. Fully accomplished Personal Data Sheet (PDS) with a recent passport-sized picture (CS Form No. 212, Revised 2017);
  3. Work Experience Sheet (CS Form No. 212);
  4. Performance Rating for the last rating period (if applicable);
  5. Report of rating/license/certificate of eligibility/eligibility card (original copy, authenticated copy, certified true copy, photocopy, scanned copy, or site/screen capture of the eligibility using the Civil Service Eligibility Verification System, Professional Regulation Commission’s [PRC] ‘s Licensure Examination and Registration Information System [LERIS];
  6. Authenticated copy of Transcript of Records and Diploma;
  7. Certificate of Seminars/Training Attended.

Applicants with INCOMPLETE documents will not be included in the evaluation. Interested and qualified applicants should signify their interest in writing.
